Greater integration of housing, health and social care is essential to improve outcomes and alleviate the unprecedented pressure on the social care sector. Yet concerns about cost and complexity are among the factors holding back wider integration. This session will demonstrate what has been achieved and how to build the case for increasing the volume of services and residents into this integrated model.

Discussion agenda:

  • Building the case for more integrated models:
    • Align your plan with a strong social vision and purpose and get the board behind it
    • How to address concerns around cost and complexity
    • What are the opportunities?
  • Learning from successful examples of integrated care services
  • How can we make good practice more consistent nationally?
  • How can we afford to do more integration?
  • The opportunity of technology and tech-enabled care to support joined up working:
    • Sharing budget and data to enable insights into both homes and tenant wellbeing
