It is often hard for GPs to know what to ask and advise, but this is a critical intervention that should be used to identify those at risk of homeless before it happens.

The Solution
Toolkit with resources for GPs to facilitate conversations with patients about homelessness. A "deck of cards" to easily ask questions that might signify risk of homelessness and identify those at risk.
